What is your cancellation policy?
We require at least 24 hours' notice for cancellations or rescheduling. Late cancellations and no-shows may be subject to a missed appointment fee.

Insurance & Billing

What insurance do you accept?
We accept Medicaid, Medicare, and most private health insurance plans. Contact your insurance provider to verify coverage, or call our billing department for assistance.
I don't have insurance. Can I still receive care?
Absolutely. We offer a sliding fee scale based on your family size and income so that everyone can access affordable healthcare.
How does the sliding fee scale work?
Complete a financial assessment form and provide documentation of your household income and family size. Your fee is then adjusted based on federal poverty guidelines. Assessments are reviewed annually.
What payment methods do you accept?
We accept cash, checks, debit cards, and major credit cards. Payment plans may be available for patients who need them.

What should I bring to my first appointment?
Photo ID, insurance card, list of current medications, immunization records, and any relevant medical records from your previous provider. Arrive 15 minutes early.
Can I transfer my medical records from my previous doctor?
Yes! Our referral specialists will handle the paperwork to request and transfer your records from your previous provider.

Patient Portal

What is the Patient Portal?
It's a secure online system where you can view your health records, test results, request prescription refills, message your care team, and manage appointments — all from your computer or smartphone.
How do I get access to the Patient Portal?
