Sliding Fee Scale — Affordable Care for Everyone
In addition to accepting Medicaid, Medicare, and private health insurance plans, we also serve individuals who are not covered by health insurance. Patients are charged for services on a sliding fee scale based on the patient's family size and income. Tejas Health Care provides healthcare everyone can afford.
To qualify for the sliding fee scale:
- •Complete a financial assessment form (available at our front desk)
- •Provide documentation of household income (pay stubs, tax return, or benefits statement)
- •Provide proof of family size (tax return or other documentation)
- •Sliding fee determinations are reviewed annually and can be updated if your financial situation changes
